Finding a great place to live is very important!

 

CLA takes this process very seriously and several people are involved to make sure our clients are in a place they want to live and have plenty of supports.

Intake Coordinator

This individual is responsible for meeting with each potential new service recipient and those who know him/her best. He/she gathers information about the individual’s needs and wants to determine if CLA can provide those services. 

At the same time the Intake Coordinator is also meeting with potential new host home providers.  He/she interviews the potential provider and goes out to his/her home to make sure that it passes required safety standards. The potential host home provider is thoroughly screened with a National and Colorado Bureau of Investigation background check, Department of Motor Vehicle check, and reference checks. In addition, we require that each host home provider have required training before they can work with a service recipient. The Intake Coordinator will also find out as much information about the potential host home providers to help ensure a great match for both client and host home provider. 

Program Management

Once someone chooses CLA for his/her residential services, he/she is assigned a Program Manager. The Program Manager will meet with the individual on a regular basis to make sure that his/her needs are being met. In most cases, the Program Manager comes out to visit each individual on a monthly basis. At that time, he/she also meets with the host home provider. The service recipient and/or his/her guardian is always welcome to contact the Program Manager by phone if there are any questions or concerns.

The Program Managers are supervised by the Residential Director. He/she oversees the residential department and makes sure that we are following all of the state regulations and to make sure we are providing quality program management services.

Nurse

CLA employs a part-time registered nurse to assist with all of the service recipients’ medical needs. The nurse is responsible for reviewing all medication changes, following up on medical problems, periodic home visits with clients, and training host home providers and staff on service recipients’ specific medical needs.

Controller

The Controller makes sure that each individual gets and keeps any benefits that are available. This may include, but is not limited to, Social Security, Railroad, and Veterans Administration.
